Client Intake Form

By signing below I confirm and acknowledge the following: 1. I am at least 18 years of age and legally capable of giving consent (Art. 13 ZGB). 2. I have truthfully disclosed all relevant medical conditions, allergies, medications and contraindications. I understand that withholding medical information may endanger my health and releases the studio from liability (Art. 100 OR). 3. I understand that tattooing / PMU involves inherent risks including but not limited to: infection, scarring, allergic reactions, keloid formation and colour migration. These risks have been explained to me. 4. I am not under the influence of alcohol, drugs or any other substances that impair my judgement. 5. I agree to follow all aftercare instructions provided by the artist. I understand that failure to do so may affect the healing and appearance of the tattoo and that the studio bears no liability for complications resulting from improper aftercare (Art. 127 OR). 6. I consent to the collection, storage and processing of my personal data — including health data and my signature — solely for the purpose of this appointment and the studio's legal record-keeping obligations, in accordance with the Swiss Federal Act on Data Protection (nDSG), in particular Art. 17, 19, 25 and 32 nDSG. I may request deletion or correction of my data at any time by contacting the studio. 7. I acknowledge that a tattoo is a permanent body modification. I accept the final design placement and size as discussed with my artist.
